Rapid Review Process Flowchart

Step 1

Within five working days of incident.

  • Any safeguarding partner might assess that a serious incident should be referred for consideration of rapid review.
  • Using the full 5 working days from the date of incident, prioritise communication with the safeguarding lead from your agency who sits on the Child Safeguarding Practice Review Group (CSPRG). Contact the Devon SCP Business team to inform them devonscp@devon.gov.uk.
  • A meeting will be set up so the case can be discussed with the Children’s Social Care, the Police, Health and Education lead representatives who will together agree whether the criterion for a Serious Incident Notification (SIN) is met .
  • Information from other partner agencies may be sought to inform decision making. Once agreed, the SIN is completed and sent National Panel and Ofsted.
  • Usually, all SINs would lead to an RR. However, if deviating from this, state in the SIN clearly why.

If the threshold is met, Rapid Review commences.

Please be aware that if the SIN is sent to National panel before midday, that day will be Day 1; if it is sent after midday, the next day counted as Day 1.

Step 2

  • Working day 1
  • Copy of SIN notification or ‘Need to Know Briefing’ sent to Devon SCP Business Team by Local Authority Notifier (this acts as the referral for the Rapid Review).
  • Letters, templates, guidance sent to Safeguarding Leads for all relevant agencies.

Step 3

  • Working day 2-9
  • Individual Agency Summaries produced by relevant agencies by relevant agencies.
  • Devon SCP informs Executive Partners that a Rapid Review is in progress and schedules ‘Exec Huddle’ for Day 14.

Step 4

  • Working day 5/6
  • A Progress Meeting for CSPRG members to report on progress of individual agency summaries and identify further agencies to contact.

Step 5

  • Working day 10/11
  • Individual Agency Summaries reviewed by a panel of Child Safeguarding Practice Review Group members.
  • Summary and Outcome Report produced including recommendations on behalf of Devon SCP Executive.
  • Immediate child safeguarding and learning should be implemented if identified by the Rapid Review.

Step 6

  • Working day 12-15
  • 1st version of the Summary and Outcome Report written and shared with Panel members for their sign off.
  • 2nd Version shared with Executives for their huddle (around day 14).
  • Amended Summary and Outcome Report sent to the Executive Safeguarding Partners for approval and sign-off (once they have huddled).

Step 7

  • Working day 16
  • Summary and Outcome Report sent to National Child Safeguarding Practice Review Panel.
  • The report is also shared with Chair of the Business Group and SWCDOP (if death) to review impact of actions to further develop practice.
  • Where a full CSPR is proposed, this is Day 0 of CSPR six-month timeline.
